St. Ann’s housing and support programs serve the diverse needs of women, children and families:

  • Teen Mother Baby Program: Providing pregnant and parenting young women, ages 13 to 21, with a safe and supportive place to prepare for motherhood, continue their education, and pave a path toward a family supporting career.

  • Supportive & Transitional Housing Program: Helping single-mother families move from homelessness to independence by addressing the root causes of their instability and guiding them toward a family-supporting career.

  • Clinical and Social Work Services: Helping residents and their families stabilize by providing clinical and social work services to improve the mental, emotional and behavioral health.

  • Education & Employment Program: Addressing the barriers to academic and career advancement for pregnant and parenting young women. Our staff works with clients to create an individualized path toward high school completion, higher education, and a family-sustaining career.

  • Child Care Center: A nurturing place where children from our community and in residence at St. Ann’s reach their fullest potential.

 
Position Summary
The Social Worker plays a critical role in supporting the mental health care of our residents. The role conducts mental health assessments to learn about each resident’s unique mental health needs and provides individual and group clinical support and therapeutic services. The Social Worker engages daily with our residents and works closely with other members of our care team to provide ongoing assessment, monitoring, and support for our residents. This role requires a Social Work license or the active pursuit of one. The Social Worker reports to the Director of Clinical and Social Work Services.
 
Essential Responsibilities:

  • Meet with new residents to conduct Mental Health Assessments and develop Mental Health Care Plans.

  • Build trusted relationships with clients and use problem-solving skills to prioritize and address their short- and long-term needs.

  • Provide therapy services (individual, group, and family).

  • Research and connect residents to community resources and social service agencies.

  • Communicate regularly with team members regarding residents’ progress and needs; coordinates team meetings.

  • Monitor progress and provides ongoing interventions.

  • On occasion, drive residents to all medical, school, and court-related appointments and meetings. 

  • Evaluate outcomes and actively participate with Program Directors and staff towards implementing program changes.

  • Prepare monthly and timely reports and case notes in the AWARDS software.

  • Serve as a liaison for family members, referring agencies, and other involved persons.

  • Develop Discharge Plans for ongoing mental health care and mental health self-care.

  • Attend relevant workshops and conferences related to program needs and individual professional development and shares relevant training with staff.

  • Participate in program and quality improvement processes by collecting and analyzing outcome data, suggesting program enhancements, and implementing identified programmatic changes.

  • Serve as a Field Instructor to interns.

  • Complete other duties as assigned.

 
Qualifications

  • Licensed Bachelor’s or Licensed Master’s degree in Social Work.

  • Licensed or working on licensure in Maryland.

  • A valid driver’s license with a clean driving record.

  • Minimum of two years’ experience serving at-risk and vulnerable adult women, teenagers, and children, including:

    • Conducting client assessments and interviews

    • Developing, monitoring, and reporting on Individual Service Plans

    • Communicating with internal teams and external community resources

    • Advocating or intervening effectively on behalf of a client

  • Authentic commitment to caring for at-risk and vulnerable populations.

  • Skilled oral and written communicator.

  • Highly organized with a proven ability to manage time well and prioritize responsibilities.

  • Demonstrated ability to engage with people from many backgrounds and cultures.

  • Strong computer skills and experience with Microsoft, databases, and Internet research

 
Schedule
The Social Worker’s schedule is below.

Monday: 10 AM - 6 PM
Tuesday: 9 AM - 5 PM
Wednesday: 11 AM - 7 PM
Thursday: 9 AM - 5 PM
Friday: 9 AM - 5 PM

Compensation
Salary: Annual targeted salary range, depending on education, experience, and licensure: $60,000 - $75,000
